Why creatine matters if you are on a GLP-1 medication

Eat more protein is the advice you hear most, and it is good advice. But there is a second one worth knowing about. Creatine is one of the few supplements with real research behind it, and it rarely comes up for people losing weight on Ozempic, Wegovy, Mounjaro, or Zepbound.

Creatine is a natural compound your muscles use for quick energy. Your body makes some on its own, and you get the rest from food, mostly meat and fish. The leading research review from the International Society of Sports Nutrition found that taking creatine alongside strength training helps people build and keep more muscle than strength training alone, and that it is one of the most well studied and safest supplements available.

Why this matters more on a GLP-1

  • You are eating less meat and fish. Meat and fish are the main food sources of creatine. Eating less overall usually means less creatine coming in from food.
  • Fast weight loss raises your muscle loss risk. Creatine paired with strength training has been shown to help people hold onto muscle during weight loss, not just build it.
  • You may already be low on fluids. Eating and drinking less can leave you a little dehydrated. Creatine pulls water into your muscles, so getting enough water matters even more once you start taking it.

How to take it

  • Take 3 to 5 grams of creatine monohydrate each day. That is the amount most research uses and it works just as well as a bigger loading dose.
  • Mix it into something you are already drinking, like a protein shake or a glass of water. You do not need to add another separate thing to your day.
  • Take it at roughly the same time each day. Creatine builds up in your muscles over a few weeks, so it does not need to be timed around workouts to work.
  • Drink a little more water than usual once you start. Creatine draws water into your muscles, so your body needs more fluid overall.

What to watch for

  • Mild stomach upset. Taking a large dose all at once can cause bloating or cramping. Taking your 3 to 5 grams with food usually clears this up.
  • A small bump on the scale. Creatine pulls water into your muscles, which can add a pound or two early on. That weight is water in your muscle, not fat.
  • Kidney health. Creatine has a long track record of being safe for people with healthy kidneys. If you have kidney disease or kidney concerns, talk to your doctor before starting.
